The medicine isn't the problem — the operational complexity surrounding it is. Billing gaps, rising A/R, and administrative overhead compound quietly until staying independent feels harder than it should.

| Payer | Claims Pending | Oldest Claim | Outstanding |
|---|---|---|---|
| BCBS | 14 | 47 days | $24,180 |
| Medicare | 9 | 31 days | $17,640 |
| Aetna | 7 | 58 days | $10,920 |
| UnitedHealthcare | 5 | 22 days | $9,350 |
April performance reflects above-benchmark results on collection rate and A/R days. Gross revenue grew 8.4% month-over-month driven by increased session volume. Denial rate (15.8%) remains the primary concern and warrants immediate attention.
Credentialing audit with BCBS and Aetna is underway — initiated before the May billing cycle. Estimated recovery: $4–8k in previously denied claims. No action needed from you.
Denial rate has increased from 11.2% in February to 15.8% in April — a 4.6-point rise over 10 weeks. The acceleration correlates with the addition of 2 telehealth modality codes in March.
1. Missing modifier (38%) — telehealth place-of-service code inconsistencies.
2. NPI mismatch (29%) — rendering provider not listed on payer roster.
3. Duplicate claim submission (18%) — billing system sync lag.
Modifier audit is underway for all telehealth claims filed after March 1. Provider NPI reconciliation with BCBS and Aetna rosters is in progress — expected resolution within 14 days. Fonx is managing this directly.
64% of outstanding A/R is current (0–30 days), which is healthy for a psychiatry practice. The 90+ day bucket at $3k (5%) is within acceptable range but should be reviewed — these are likely write-off candidates or require secondary payer follow-up.
BCBS claims older than 47 days have been escalated by Fonx. Three claims totaling $8,200 exceeded the standard adjudication window — formal escalation submitted. You'll see updates reflected in next month's A/R report.
Provider A (LCSW): Not listed on BCBS telehealth roster. Claims submitted after March 1 are being denied at 94% rate. Estimated revenue at risk: $6,400/month.
Provider B (NP): Aetna credentialing application submitted Feb 12 — no response received. Pending 52 days. Follow-up required.
Corrected roster submitted to BCBS for Provider A — under review. Provider B's Aetna application has been escalated via provider relations (pending 52 days, beyond the 45-day SLA threshold). Fonx is managing both resolutions directly. No action needed from your team.
